Diverge Supports is proud to support Adelaide’s NDIS community through a nurse-led, person-centred approach to disability care.
We understand that each condition brings unique challenges, and that tailored supports make all the difference. Below are the main conditions we specialise in supporting.
MS affects the central nervous system, often causing fatigue, muscle weakness, mobility changes, balance issues, and cognitive impacts.
At Diverge Care, our nurse-led team assists with symptom management, mobility supports, therapy coordination, and daily living assistance to maintain independence and quality of life.
After a stroke or brain injury, participants may experience changes in movement, speech, memory, coordination, or emotional control.
We deliver rehabilitative supports, cognitive assistance, nursing oversight, and community access services to help with recovery, rebuilding function, and reconnecting with life.
CP is a neurological condition affecting movement, muscle tone, and posture, often from birth. It may also affect speech, coordination, and daily living.
We offer mobility assistance, assistive technology, therapy support, home modifications, and nursing care to help participants live more independently with dignity.
Autism influences communication, social interaction, sensory processing, and routines. Strengths may include deep focus, pattern recognition, and strong memory.
Our supports span skill development, sensory-aware care, community participation, therapy, and environment adaptations, always respecting the individual’s preferences and pacing.
Brain Injury
Brain injury can occur after trauma, stroke, lack of oxygen, or illness, and may lead to lasting changes in memory, concentration, behaviour, emotions, movement, or speech.
At Diverge Supports, our nurse-led team provides personalised support that may include rehabilitation assistance, daily living support, behaviour strategies, and community re-engagement. Our goal is to help participants rebuild skills, maintain independence, and live confidently after injury.
Parkinson’s Disease
Parkinson’s is a progressive neurological condition marked by tremors, stiffness, balance challenges, and mobility decline.
Our nurse-led clinical supports, allied therapies, mobility aids, and daily living assistance help slow decline, manage symptoms, and maintain quality of life.
Intellectual Disability
An intellectual disability affects how a person learns, understands information, and applies new skills in daily life. It can impact areas such as communication, problem-solving, and social understanding. Intellectual disability is usually present from childhood and can range from mild to more significant levels of support needs. With the right understanding and tailored care, people with intellectual disabilities can lead active, meaningful lives within their communities.
