A participant isn’t a Support Coordination client on Monday, a support work client on Tuesday and an NDIS participant on Wednesday.

They’re the same person.

They have their own interests, relationships, routines, challenges, strengths and goals. Their support needs don’t exist in neat categories, and their life certainly doesn’t operate according to the divisions between different services.

Someone might need help understanding their NDIS plan, finding the right providers, attending appointments, building independence at home and getting involved in their community.

These things are connected.

When the people supporting someone understand the bigger picture, it becomes easier to make sure those supports are actually working towards the same outcomes.

Support should be about outcomes, not just services

It’s easy for disability support to become focused on what a service provides.

How many hours?

Which support?

Which provider?

What does the plan say?

What can be claimed?

These things are important, but they aren’t the whole picture.


The more important question is:

What does this person want their life to look like?

Perhaps they want to become more independent at home.

Perhaps they want to build confidence using public transport.

Perhaps they want to find meaningful employment.

Perhaps they want to reconnect with their community.

Perhaps their family needs more support so they can maintain their own wellbeing.

Or perhaps the goal is simply to make everyday life feel a little more manageable.

The role of good support is to help turn those goals into practical steps.

When everyone works together, support becomes simpler

A participant may have a Support Coordinator, Support Workers, allied health professionals, nurses, family members, carers and other services involved in their support.

When communication is strong, these people can work together rather than working in isolation.

A Support Coordinator can help understand the bigger picture, identify gaps, connect people with appropriate services and help navigate the NDIS.

A Support Worker may be supporting the person to put those goals into practice in everyday life.

An allied health professional might provide strategies to build skills or independence.

A nurse may contribute clinical knowledge and support around health needs.

Each person has a different role — but the participant should remain the common thread connecting them all.

The goal isn’t for everyone to do the same thing. It’s for everyone to understand where they’re heading.

Why we’ve brought our services together

For several years, A1 Coordination and Diverge Supports have operated as separate businesses, providing different parts of the support journey.

From 1 September 2026, they are becoming one name: Diverge Care.

The change reflects something we have always believed: support works best when it is connected.

By bringing Support Coordination and Support Work together under one brand, we can create a clearer, more streamlined experience for participants, families and support networks.

It doesn’t mean that every participant needs every service.

It means that when someone does need different types of support, there is an opportunity for those services to work together with a shared understanding of the person and their goals.

Your goals should guide your supports

Good Support Coordination isn’t about telling someone what they should do.

It’s about helping them understand their options, make informed decisions and build the support network that works for them.

Good Support Work isn’t about simply completing a list of tasks.

It’s about supporting someone to develop skills, participate in their community, maintain their independence and live their life in the way they choose.

Both approaches start with the same thing:

Listening to the person.

Because the best support isn’t necessarily the support with the most services.

It’s the support that makes sense for that individual.

Less complexity. More connection.


We know that navigating disability services can sometimes feel overwhelming.
